Medicare Part B covers acupuncture only for chronic low back pain. It pays for up to 12 sessions in 90 days, then 8 more if you’re getting better, for a maximum of 20 sessions in 12 months (Medicare.gov). You pay 20% of the Medicare-approved amount after the $283 Part B deductible.

For any other problem, like knee arthritis, neck pain, migraines or nerve pain, Original Medicare won’t pay. That rule also covers dry needling. The hard part is finding a provider who can bill Medicare. Here’s how it works.

What counts as chronic low back pain

Medicare uses a narrow definition. Your low back pain must (CMS NCD 30.3.3):

  • Last 12 weeks or longer
  • Have no known cause. It can’t be from cancer that has spread, an inflammatory disease or an infection.
  • Not be linked to surgery
  • Not be linked to pregnancy

So if your back pain is from a recent surgery or a known disease, Medicare won’t cover acupuncture for it. CMS says acupuncture for any condition other than chronic low back pain is not covered.

Table of Medicare acupuncture rules in 2026
Medicare's acupuncture rules, visit limits and costs in 2026. Chart: SeniorsSecrets.com, from Medicare.gov, CMS and KFF.

The visit limits

  1. First round: up to 12 visits in 90 days.
  2. If you improve: 8 more visits.
  3. Yearly cap: no more than 20 acupuncture treatments in a 12-month period.

If you’re not improving after the first round, Medicare stops paying. Medicare.gov says you’ll pay 100% if you keep going. Ask your provider to write down your progress, such as pain scores and what you can do now that you couldn’t before. That record is what supports the extra 8 visits.

Who can give Medicare-covered acupuncture

This is where most people get stuck. Under the law, licensed acupuncturists can’t bill Medicare directly. CMS allows acupuncture from:

  • Doctors (physicians), following state rules
  • Physician assistants (PAs), nurse practitioners (NPs) and clinical nurse specialists (CNSs) who have:
  • a master’s or doctoral degree in acupuncture or Oriental Medicine from a school accredited by ACAOM, and
  • a current, full, unrestricted state acupuncture license
  • Auxiliary personnel, like a licensed acupuncturist, who meet those same degree and license rules and work under the right level of supervision by a doctor, PA or NP/CNS

In plain words: a licensed acupuncturist can treat you under Medicare only with a doctor or NP who supervises and bills.

My advice: Before your first visit, ask the clinic two questions:

  1. “Will this visit be billed to Medicare under a doctor, PA or NP?”
  2. “Do you accept Medicare assignment?”

If the answer to the first question is no, you’ll pay the full cost yourself.

What you pay in 2026

With Original Medicare:

  • The $283 Part B deductible, if you haven’t met it this year
  • 20% of the Medicare-approved amount for each visit
  • 100% for visits past your limit, visits for other conditions, or visits after Medicare finds you’re not improving

A Medigap plan usually pays your 20% for covered visits. It won’t pay for visits Medicare doesn’t cover. See our Medigap plans comparison.

If a clinic thinks Medicare won’t pay for a visit, it may ask you to sign an Advance Beneficiary Notice (ABN). That’s a form that says you agree to pay if Medicare says no. Read it before you sign.

Medicare Advantage plans

Medicare Advantage plans must cover acupuncture for chronic low back pain the same way Original Medicare does. Many also add more as an extra benefit. KFF found 34% of individual Medicare Advantage plans offer acupuncture as a supplemental benefit in 2026 (KFF).

These extras vary. A plan may allow a set number of visits for any condition, or require you to use certain providers. Check your plan’s Evidence of Coverage under “acupuncture.” If acupuncture matters to you, compare it during Open Enrollment, October 15 to December 7, 2026.

Does acupuncture help back pain?

For back pain, the research is fairly good. The NIH’s center for complementary health (NCCIH) points to a 2017 guideline from the American College of Physicians. It listed acupuncture as a first choice, non-drug option for chronic low back pain. The evidence for chronic low back pain was judged moderate quality (NCCIH).

NCCIH also says few problems have been reported, but serious harm can happen with non-sterile needles or poor technique. The FDA requires acupuncture needles to be sterile and for single use.

Who should NOT count on Medicare acupuncture

  • People whose back pain is from a known cause, like a fracture, cancer or recent surgery
  • People who want acupuncture for neck pain, knees, headaches or nerve pain (Original Medicare won’t pay)
  • People who see an acupuncturist who doesn’t work with a Medicare-billing doctor or NP

Safety check: Tell your doctor and the acupuncture provider about all your health problems and medicines, including blood thinners. Have new or worsening back pain checked by a doctor first.

Other back pain care Medicare covers

Acupuncture isn’t your only option. Medicare Part B also covers physical therapy when medically needed, and chiropractic spinal manipulation to correct a subluxation (a spine bone out of position). Our Medicare chiropractic coverage guide explains those limits. If you think your plan wrongly denied a visit, our Medicare appeals guide shows how to fight it.

Steps to get covered acupuncture

  1. See your doctor and confirm your back pain fits Medicare’s chronic low back pain definition. Ask them to note it in your chart.
  2. Find a qualified provider who bills Medicare through a doctor, PA or NP.
  3. Confirm assignment and your cost per visit.
  4. Track your progress during the first 12 visits.
  5. Ask about the next 8 visits before visit 12 if you’re improving.

Common questions

Does Medicare cover acupuncture for neck pain or knee pain?

No. Original Medicare covers acupuncture only for chronic low back pain. Some Medicare Advantage plans cover more as an extra benefit.

How many acupuncture visits does Medicare cover?

Up to 12 in 90 days, then 8 more if you’re improving, with a maximum of 20 in 12 months.

Can a licensed acupuncturist bill Medicare?

Not directly. A licensed acupuncturist can treat Medicare patients under the supervision of a doctor, PA or NP who bills Medicare, if they meet CMS degree and license rules.

Does Medicare cover dry needling?

Only for chronic low back pain, under the same rules as acupuncture. CMS treats dry needling as a type of acupuncture.

How much does acupuncture cost with Medicare?

You pay 20% of the Medicare-approved amount after the $283 Part B deductible in 2026 for covered visits.
